I understand that part of the program at Prime Foundation Montessori involves taking children off campus to explore the area surrounding the school grounds and in Montgomery County. I hereby permit and consent to my child leaving campus during school hours and afterschool (PrimeCare and PrimePlus) programs to visit programs, events and places in the neighborhood within walking distance including but not limited to the Playground, the local library, the local Fire Station, Recreational centers, the nearby park and playgrounds, the local store to purchase flowers, and a walk to collect fall leaves. I understand that my child will be with other children and will be accompanied by a member of the Prime staff and at least one other adult. I will not receive advance notice for these planned and spontaneous excursions and other programs that occur during the school day.

I voluntarily and willingly assume responsibility for all risks and dangers associated with my child's participation in going out occasions. In consideration of my child's participation, I waive all claims or causes of action against Prime Foundation Montessori, its officers, directors, employees, and agents arising out of my child's participation in the going outs, and hereby release, hold harmless and discharge Prime Foundation Montessori and its officers, directors, employees, and agents from all liability, except for gross negligence or willful misconduct.

• A parent or legal guardian who does not so agree must notify in writing the Head of School (info@pfmschool.org) by November 1st of this School Year. • Only the registered parent or guardian is allowed to complete this form. • You MUST provide your parent code and your signature for this Consent and Waiver form to be accepted by the School.
