I consent to my child’s participation in various classroom field trips through the current school year. My child is in good health and has no physical condition that would prevent participation. I understand that some field trips may include my child being driven by a staff member or another parent or accompanied by parents and staff in a vehicle or car, on a bus or Metro. I understand that the School will endeavor to give 24 hours’ notice of field trips, but that the notice may be shorter or non-existent on rare occasions. All field trips will include one or more teachers and one or more teacher’s assistants or staff.

In consideration of my child's participation, I waive all claims or causes of action against Prime Foundation Montessori, its officers, directors, employees, and agents arising out of my child's participation in the going outs, and hereby release, hold harmless and discharge Prime Foundation Montessori and its officers, directors, employees, and agents from all liability, except for gross negligence or willful misconduct.
