I consent and permit my child to leave campus via car, on a bus, school parent’s vehicle, staff vehicle, metro or walking in order to participate with a small group of peer students in going outside of the School to follow an interest, conduct an interview, and explore the area for research purposes. These are referred to as “going outs.” I understand that my child will be accompanied by at least one adult (Prime staff member or parent) on these going outs, and that they are an essential component of the Montessori curriculum and philosophy. The School will endeavor to provide 24 hour notification of going outs but some of these may occur without advance notification.

I voluntarily and willingly assume responsibility for all risks and dangers associated with my child's participation in going out occasions. In consideration of my child's participation, I waive all claims or causes of action against Prime Foundation Montessori, its officers, directors, employees, and agents arising out of my child's participation in the going outs, and hereby release, hold harmless and discharge Prime Foundation Montessori and its officers, directors, employees, and agents from all liability, except for gross negligence or willful misconduct.

• If a parent or legal guardian does not agree, they must notify the Head of School in writing (info@pfmschool.org) by November 1st of this school year. • Only the registered parent or guardian is allowed to complete this form. • You MUST provide your parent code and your signature for this Consent and Waiver form to be accepted by the School.
